Effective immediately, physicals and all appropriate documents for tryouts should be connected or linked to Rank One
You can still turn your physicals into the front office at LKMS, but all waiver and insurance forms MUST be completed on RankOne before participating in athletics.

Do ALL RANK ONE ACCOUNT INFORMATION FIRST, and then take the HISTORY FORM to the Doctor to get your Physical Examination Form complete.
